At Valor Mentoring, we have a heart for the people in our community. We strive to offer a variety of opportunities to come alongside others through mentoring, recreation, special interests and events that can offer encouragement and value to one's life journey.

Volunteer Application

Valor Mentoring exists to provide mentorship to those in need through one on one relationships, utilizing music, media & arts, and community outreach projects.

Statement of Non-Discrimination:

Expectations:
· To support the mission and vision of Valor Mentoring.
· To follow the guidelines and procedures set forth by Valor Mentoring.
· To treat all Valor Mentoring staff, clients, and volunteers with respect and dignity at all times.

· Volunteers will be held to the same operational standards, policies, and client confidentiality as paid staff.
Volunteers not following the above requirements and expectations are subject to dismissal after review by the Valor Mentoring leadership.

If yes, please explain below. (Note: Answering yes will not automatically prohibit individuals from becoming volunteers, but will be considered with respect to time, circumstances, seriousness, and relationship to volunteer responsibilities. Some volunteer positions may require a background check. If you are selected for one of those assignments, you will be provided with a separate criminal background check authorization form.)

Parental Consent (if minor)

I give my consent for my child, named on page one of this application, to provide volunteer services to Project Understanding. I also give Project Understanding my consent to obtain any emergency medical treatment necessary for the safety of my child.

Application Agreement

My signature below certifies that all statements made on this application are true, complete, and correct to the best of my knowledge and belief. I understand these statements are subject to verification. I understand that falsification on this application can disqualify me from consideration or result in my volunteer services being denied. Furthermore, my signature below provides my authorization to Valor Mentoring to conduct driver's license and motor vehicle record checks as needed, as well as reference checks to determine my suitability for placement.
